WORD OF FAITH ACADEMY

Admission and Requirements

Parents interested in enrolling their children in Word of Faith Academy must act according to the following procedures:

1. Complete a Statement of Interest

2. Acquire a registration packet and application

3. Turn in completed application packet and application to WOFA

4. Schedule a PAS (Parent-Administrator-Student) Interview

**Application packet includes the following:

1. Form 121 and/or up-to-date shot records

2. Original birth certificate

3. Original social security card

4. Two proofs of residency

5. Three contact numbers for parents

6. Proof of personal insurance

7. Results from recent physical examination

8. List of allergies and allergic reactions

9. Complete medical history

10. Entrance examination (To be given at interview)

11. Completed application packet

12. Withdrawal forms from previous school

13. Behavioral and academic records from previous school

14. Volunteer hours

***All items must be turned in with the application packet. Original documents will be returned. Once completed packet has been received, an administrator will contact the parents to schedule a PAS interview. Failure to interview on originally-requested date could lead to the child's name being placed at the bottom of the waiting list. Please note, completing an application packet does not guarantee acceptance into the academy.
